Explore a SafeWave Assessment Questionnaire

Examine one AI-enabled system, workflow, product, business unit, or deployment privately. You can use the questionnaire on its own and decide later whether to share anything or request a report.

Acceleration-enabling containment for complex and evolving systems.

SafeWave helps identify where instability, escalation, or loss of control may emerge as autonomy, interaction, authority, and execution speed increase.

The objective is not slowdown. The objective is bounded acceleration.

Use the assessment privately—even if your work is sensitive or high-consequence

Beginning a questionnaire does not begin an engagement, transmit your answers, or require you to identify yourself, an organization, or a system.

Free to run. No login. No email. No identification required. You do not have to provide your name or identify your employer, organization, model, system, project, or deployment. Your responses remain in your browser unless you deliberately choose to send them. You may keep your own copy without submitting anything by using the questionnaire’s PDF download control or your browser’s Print command and choosing Save as PDF.

Useful even if you never submit anything

The questions themselves can expose overlooked critical assumptions, interacting risk pathways, unclear authority or ownership, missing evidence, unstable behavior, and undefined recovery conditions. You may assess a real, planned, hypothetical, composite, or anonymized system and keep every response in your browser.

Request the free report without identifying yourself

No personal or work email address is required. SafeWave does not require or verify your identity, employer, organization, or system name.

To receive a report, only a return address and the automatically generated Assessment Reference ID are needed. You decide what information to submit and may use generic system labels, anonymized descriptions, hypothetical scenarios, or omit identifying details.

Need a non-identifying return address?

Create a separate address with a privacy-focused email provider such as Tuta. Choose an address containing no personal, employer, organization, project, or system name—for example, assessment7k4m@tuta.com. Use that address only to request and receive the report. Avoid short-lived disposable addresses that may expire before the report arrives.

Identity is not required. Disclosure remains under your control. Avoid including distinctive details that could identify you or your organization if that is a concern.

Choose how your report is written. Your free report may use SafeWave architecture names or neutral functional terminology. Neutral functional terminology is used if no choice is recorded. The findings, evidence, seriousness, and recommendations remain the same; only the terminology changes. You may use the report independently whether or not you ever contact SafeWave again.

We remove these barriers because SafeWave’s mission is to help make AI safe for humanity.

Why SafeWave offers this

Our primary purpose is to help make advanced AI safer for humanity. The assessment also helps organizations determine whether SafeWave’s engineering capabilities may address risks their present controls do not.

If you find the assessment or report valuable, you may later choose to explore becoming a SafeWave customer or partner. There is no obligation, and using the assessment privately or requesting a report will not place you in a sales process.

SafeWave will not use a report-delivery address for promotional contact unless you expressly request further communication. You would need to identify yourself only if you voluntarily choose to begin a customer or partnership discussion. Any later confidential technical exchange, agreement, or payment would occur through a separate secure process.

Your choices remain separate. Using the questionnaire, submitting information, requesting a report, and proceeding into implementation are four distinct decisions. You control each one.

Choose the questionnaire that fits your system

These are two questionnaire formats, not commitments to an assessment pathway. Each can provide value on its own. A report or further SafeWave engagement occurs only if you deliberately choose to proceed.

Not sure which questionnaire to use?

Start with the Business AI Risk Questionnaire for ordinary business AI use. It can help reveal whether the full system questionnaire may be appropriate because of deeper execution, propagation, autonomy, financial, customer-facing, physical, or high-consequence exposure.

What the questionnaires examine

The questionnaires help identify where escalation, instability, or loss of control may emerge within a system and where stronger execution boundaries may be needed.

SafeWave examines whether boundaries around execution, authority, propagation, memory, resource demand, human influence, physical action, and future capability growth remain enforceable as systems scale.

The questions surface where behavior is generated, how actions are executed, how effects propagate, and where persistence or drift may occur.

Important scope

Complete one questionnaire for a single system, product, business unit, workflow, or use-case.

Organizations often operate multiple products, services, platforms, business units, or deployment environments. Each may exhibit distinct escalation dynamics and require different control boundaries.

How the questionnaire can lead to an optional assessment

SafeWave is an evolving preventive AI systems architecture whose established components operate at distinct control boundaries. Each component addresses a defined point where escalation or loss of control can occur.

SafeWave controls can operate within runtime, platform, infrastructure, firmware, hardware, or silicon environments, depending on the boundary being governed and the assurance required.

SafeWave does not require rewriting or replacing existing systems. It introduces bounded execution, propagation, and interaction controls alongside existing architecture.

If you deliberately submit a completed questionnaire and request a report, SafeWave can map the information you provide to structural control boundaries:

The resulting assessment identifies where containment may be applicable — not which products you must adopt.

Optional report pathways

Business AI Risk Snapshot Report

The Business AI Risk Snapshot is an optional lighter report for organizations adopting AI tools, business automation, customer-facing AI, or basic agentic workflows without operating advanced AI infrastructure or high-consequence systems.

It focuses on business risk, AI exposure, oversight gaps, connected systems, and whether a deeper system assessment may be appropriate.

For small and medium-sized businesses, implementation support may eventually be handled through SafeWave-aligned cybersecurity firms, managed service providers, IT advisors, compliance advisors, or AI-governance partners. The snapshot report can serve as a practical starting point for those discussions.

Full SafeWave System Assessment Report

The full SafeWave System Assessment is an optional deeper report for systems where execution, autonomy, propagation, persistence, resource demand, physical action, financial authority, or future capability growth may create structural risk.

This includes advanced AI systems, model providers, agent platforms, infrastructure environments, robotics, financial systems, critical infrastructure, cybersecurity or cyber operations, biological or life-sciences systems, nuclear or defense-relevant systems, and other high-consequence domains.

The full assessment supports deeper component-level recommendations and may route to high-consequence follow-up questionnaires where additional domain-specific context is needed.

What you may receive if you request a report

The report type depends on the questionnaire used, the information deliberately submitted, and the report requested.

Business AI Risk Snapshot Report

Full SafeWave System Assessment Report

The full assessment report is designed to identify what the system needs before implementation work begins. It does not deliver engineering specifications at the assessment stage.

Each recommendation includes a clear explanation of the risk or gap it addresses, why it applies to the system described, and the boundary at which it operates. A SafeWave component is named only when the mapping is supported.

Recommendations are directly traceable from system behavior to escalation surfaces, structural gaps, and containment boundaries.

Beyond an optional assessment

Organizations that later choose to proceed may receive licensed Level Four Engineering Packs or other authorized engineering materials for recommended SafeWave components.

These follow-on materials are separate from the questionnaire and assessment report. The report identifies what may be recommended; a separate follow-on engagement supports deployment and integration planning.

Important clarification

Systems can be highly reliable under normal operating conditions and still be structurally unbounded under certain conditions. The questionnaire and any resulting assessment do not assume system failure.

They examine how behavior may change under:
